Coming for a first visit? No problem! Here is a list of everything you need to bring to your first visit!

• Insurance Cards (mandatory) with co-pay due at the time of service.
  

• Previous X-ray or MRI results; including image disc.
  

• Medical records from previous Orthopedic physician and/or last notes from your Primary Care physician.

• Current medications; including dosages, quantity, prescribing physician, last date received.
  

• Complete patient forms. (You can get these by contacting our office)
